Warren Park Nursing Pavilion is located in a quiet, residential neighborhood in West Rogers Park, next door to the Chicago Park District’s Warren Park and John Black golf course, and has been providing Skilled and Intermediate nursing care and rehabilitation for over thirty years.

Warren Park is one of the smaller facilities in the area thereby providing the residents and their families a more personalized approach to care. Board certified therapists, available twenty-four hours daily, staff our rehabilitation department. Healthcare professionals, many of whom have served the residents and the facility for numerous years, staff our facility. Rooms are graciously appointed. The out door patio and garden serves as a warm weather meeting place for residents and their families and friends. The facility van provides convenient transportation to stores, restaurants, museums, and other recreational and educational points of interests.
