Columbus Park Nursing & Rehabilitation Center is a 216 bed skilled nursing facility. In addition to being Medicaid certified, we have 108 certified Medicare beds. We can provide exceptional wound care, IV therapy, trach care, and top-rated occupational, physical, and speech therapies. Our nursing staff are here to serve each resident's needs with care, compassion, and understanding. We are readily accessible to public transportation.
Columbus Park Nursing Center is a unique facility in that we also specialize in treating older patients with both medical and behavioral or psychiatric issues through the ADAPT Program. Our psych-social staff are well-trained in behavioral interventions, crisis prevention, and meeting the mental health as well as medical needs of our residents.
At Columbus Park Nursing Center our interdisciplinary team is personally involved in each resident's care and programming. Our staff are knowledgeable about each individual's needs. Our department heads are also available 24 hours/day, 7 days/week for assistance.
